Mending Homes Care Services, LLC, comes into your home to meet with the family.
We obtain all necessary information pertaining to your child's need.
With your consent, we submit all documentation to the GAPP team for approval.

Care Plan

After all paperwork is approved through GAPP, we come up with a Care Plan for the individual client.
We then review the plan with the client, and once both parties agree, service will begin.

Caregiver

Caregiver

Caregivers are picked based off the clients needs, depending if you need skilled/ unskilled services.
Caregivers will provide services that have been agreed upon with the client, and Mending Homes Care Services, LLC.
If there is a situation where the client is not happy with the caregiver, we can always match with another caregiver.
